MuscleRésumé : Background: The path to recovery of muscle strength and mobility following discharge from the intensive care unit (ICU) has not been well described.
Objective: The study objective was to quantify muscle function, gait, and postural control at 3 and 6 months after discharge in people who were recovering from critical illness and who were ventilated for 7 days or more.
Design: This was a nested longitudinal study with continuous inclusion of individuals over a 2-year period and with age- and sex-matched controls.
Methods: Twenty-four people were tested at 3 months after ICU discharge; 16 of them (67%) were reevaluated at 6 months (post-ICU group). Healthy controls (n = 12) were tested at a single time point. Muscle function of the knee extensors (KEs), plantar flexors (PFs), and dorsiflexors (DFs) was assessed on a dynamometer. Gait was measured using an electronic walkway, and postural control was measured with 2 portable force plates.
Results: Muscle weakness was observed across all muscle groups at 3 months, with the greatest strength reductions in the ankle PFs (45%) and DFs (30%). Muscle power was reduced in the PFs and DFs but was not reduced in the KEs. Gait in the post-ICU group was characterized by a narrower step, longer stride, and longer double-support time than in the controls. Improvements were found in KE strength and in stride time and double-support time during gait at 6 months. Leg muscle strength and power had moderate associations with gait velocity, step width, and stride length (r = .44–.65).
Limitations: The small heterogeneous sample of people with a high level of function was a limitation of this study.
Conclusions: Muscle strength and power were impaired at 6 months after ICU discharge and were associated with gait parameters. Hanche , Membre inférieurTags : axe fonctionnel malalignement maltracking moment abducteur de hanche syndrome fémoro-patellaire Résumé : La littérature scientifique nous apporte des éléments nouveaux sur la prise en charge du syndrome fémoro-patellaire en rééducation. En effet, la modification des axes fonctionnels du genou induit par des déficits neuromoteurs (moment abducteur de hanche) en font une pathologie systémique du membre inférieur.
La perturbation de l'alignement dynamique (malalignement) du membre inférieur associé à des défauts d'engagement patellaire (maltracking), nous pousse à réfléchir à une évaluation plus objective afin de déterminer les facteurs responsables des pathologies patellaires et ainsi optimiser notre approche holistique du patient.

Methods: We included 21 individuals with unilateral cerebral palsy. 2PD thresholds were evaluated on thumb pads, and activation of the somatosensory cortex was recorded by functional MRI (fMRI) during passive movements of the affected hand. A lateralization index (LI) was calculated for the primary sensory (S1) and secondary sensory (S2) cortices and the correlation between the LI and 2PD thresholds was analysed.
Results: We found a significant negative correlation between the 2PD thresholds and the S2 LI (r=−0.5, one-tailed P -value=0.01) and a trend towards a negative correlation with the S1 LI (r=−0.4, one-tailed P -value=0.05).
Conclusion: High levels of activation in the contralesional hemisphere were associated with high levels of sensory impairment in individuals with unilateral cerebral palsy. Os , PiedTags : Prolifération ostéochondromateuse paraostéale bizarre Lésion de Nora Pied Récidive Résumé : La lésion de Nora est une tumeur osseuse bénigne et rare à potentiel de récidive élevé. Nous rapportons un tel cas unique de 1ère récidive à 11 ans postopératoire. Il s’agit d’une femme âgée de 42 ans qui a consulté en 2003 pour une tuméfaction dure, de 2 cm de grand axe, de la 1ère phalange du 2e orteil gauche. L’exploration histologique avait conclu à une prolifération ostéochondromateuse paraostéale bizarre. Après exérèse, elle a récidivé 11 ans plus tard.
