CerveauTags : Spatial navigation Virtual reality Brain-damaged patients Ageing Stimuli Aid Résumé : Introduction : Spatial navigation, which involves higher cognitive functions, is frequently implemented in daily activities, and is critical to the participation of human beings in mainstream environments. Virtual reality is an expanding tool, which enables on one hand the assessment of the cognitive functions involved in spatial navigation, and on the other the rehabilitation of patients with spatial navigation difficulties. Topographical disorientation is a frequent deficit among patients suffering from neurological diseases. The use of virtual environments enables the information incorporated into the virtual environment to be manipulated empirically. But the impact of manipulations seems differ according to their nature (quantity, occurrence, and characteristics of the stimuli) and the target population.
Methods : We performed a systematic review of research on virtual spatial navigation covering the period from 2005 to 2015. We focused first on the contribution of virtual spatial navigation for patients with brain injury or schizophrenia, or in the context of ageing and dementia, and then on the impact of visual or auditory stimuli on virtual spatial navigation.
Results : On the basis of 6521 abstracts identified in 2 databases (Pubmed and Scopus) with the keywords « navigation » and « virtual », 1103 abstracts were selected by adding the keywords “ageing”, “dementia”, “brain injury”, “stroke”, “schizophrenia”, “aid”, “help”, “stimulus” and “cue”; Among these, 63 articles were included in the present qualitative analysis.
Conclusion : Unlike pencil-and-paper tests, virtual reality is useful to assess large-scale navigation strategies in patients with brain injury or schizophrenia, or in the context of ageing and dementia. Reductions in lower-extremity muscle mechanical function (e.g., muscle strength) appear to have critical implications for lower-limb functional capacity. However, no review has summarized the current knowledge about the importance of muscle strength for functional tasks in PwMS. Expanding the current knowledge would advance the design of both clinical and research interventions aiming to improve functional capacity in PwMS.
Objectives: (1) To identify studies that measured lower-extremity muscle mechanical function and lower-limb functional capacity outcomes in PwMS, and (2) to map associations between muscle strength and functional capacity.
Methods: This review was based on a literature search (databases: PubMed, Embase). Included studies had to report data on lower-extremity muscle mechanical function and lower-limb functional capacity outcomes in PwMS. The associations between muscle strength and functional capacity were analyzed by using the reported correlation coefficients (R) recalculated to the determination coefficient R2. Randomized trials and observational studies were included.
Results: A total of 59 articles were reviewed; 17 (773 participants) reported associations between muscle strength and functional capacity. Lower-extremity muscle mechanical function explained a significant part of the variance in most lower-limb functional capacity tests (approximately 20–30%). This was particularly evident in muscle strength from the weakest leg. Muscle strength was predominantly tested on knee extensors and knee flexors by using isokinetic dynamometry during maximal isometric (0°/s) and dynamic (30–60°/s) contractions. Walking tests such as the timed 25-Foot Walk Test and 10-Min, 2-Min and 6-Min Walk Test were the most frequently performed functional capacity tests.
Conclusions: In PwMS, muscle strength of particularly the weakest limb explains 20% to 30% of the variance across a number of lower-limb functional capacity tests. Pays-BasTags : Migraine chronique de tension thérapie manuelle mécanisme de travail Résumé : Objectif : Pour explorer le mécanisme de travail de thérapie manuelle, nous avons examiné si 3 variables de la colonne cervicale sont modérateurs de l'effet de la thérapie manuelle sur la fréquence des maux de tête.
Contexte : La thérapie manuelle a montré la réduction de la fréquence des migraines chroniques de tension. Dans quelle mesure le fait que les éléments spécifiques de traitement contribuent à l'efficacité de la thérapie manuelle est inconnu.
Méthodes : Cent quatre-vingt-deux participants souffrant de migraine ont participé à une étude longitudinale prospective : 142 ont subi une thérapie manuelle et 40 ont reçu les soins habituels par leur médecin généraliste. L'analyse de régression a été réalisée selon la procédure décrite par Baron et Kenny, et la proportion d'effet modérateur a été estimée à 3 paramètres potentiels : (1) amplitude du mouvement cervical, (2) endurance des fléchisseurs du cou, et (3) positionnement avant de la tête. Le résultat a montré une réduction de 50% ou plus de la fréquence quotidienne des maux de tête.
Résultats : L'endurance du fléchisseur du cou a modéré l'effet de la thérapie manuelle de 24,5 %. L'amplitude du mouvement cervical et le positionnement avant du cou n'ont pas montré d'effet modérateur.
Conclusion : L'endurance croissante du fléchisseur du cou semble être un résultat du mécanisme de la thérapie manuelle. Cette constatation étaye la formation isométrique des fléchisseurs du cou chez les participants souffrant de migraine chronique de tension.
