DécretTags : activité physique adaptée ald soins de ville Résumé : Le décret du 30 décembre 2016 paru au Journal Officiel le 31 décembre 2016 permet la dispensation de l'activité physique adaptée (APA) prescrite par le médecin traitant à des patients atteints de pathologies longue durée (ALD).
L'APA comprend l'ensemble des mouvements corporels produits par contraction des muscles squelettiques entraînant une augmentation de la dépense énergétique au-dessus de la dépense de repos. Elle comprend tous les mouvements de la vie quotidienne, y compris ceux effectués lors des activités de travail, de déplacements, domestiques, ou de loisirs.
- Libérer les articulations des membres supérieurs et inférieurs tout en renforçant le système musculaire,
- Surveiller l'essoufflement, la prise de poids, la tension artérielle, le pouls,
- Prodiguer des conseils alimentaires et d'hydratation, conseils éducatifs sur les méfaits du tabac, de l'alcool, de la drogue, du stress, de l'apnée du sommeil, favorisant l'observance,
- Rassurer le patient et son entourage et ainsi l'encourager dans son activité physique,
- Optimiser le réentraînement à l'activité, permettre le déplacement et marcher dans l'appartement, puis, dès que possible, à l'extérieur,
- Contacter le cardiologue ou le médecin généraliste en cas d'aggravation de la pathologie cardio-vasculaire.
Tous ces éléments alimentent le BDK. Activité physique , Sport intensifTags : High-intensity interval training Continuous aerobic exercise training Exercise prescription Coronary heart disease Résumé : Recently, high-intensity interval training (HIIT) has emerged as an alternative and/or complementary exercise modality to continuous aerobic exercise training (CAET) in CHD patients. However, the literature contains descriptions of many HIIT protocols with different stage durations, nature of recovery and intensities. In this review, we discuss the most recent forms of validated HIIT protocols in patients with coronary heart disease (CHD) and how to prescribe and use them during short- and long-term (phase II and III) cardiac rehabilitation programs. We also compare the superior and/or equivalent short- and long-term effects of HIIT versus CAET on aerobic fitness, cardiovascular function, and quality of life; their efficiency, safety, and tolerance; and exercise adherence. Short interval HIIT was found beneficial for CHD patients with lower aerobic fitness and would ideally be used in initiation and improvement stages. Medium and/or long interval HIIT protocols may be beneficial for CHD patients with higher aerobic fitness, and would be ideally used in the improvement and maintenance stages because of their high physiological stimulus. Finally, we propose progressive individualized models of HIIT programs (phase II to III) for patients with CHD and how to ideally use them according to the clinical status of patients and phase of the cardiac rehabilitation program. FemmeRésumé : Background: Physical fitness is a marker of health and is associated with health-related quality of life (HRQoL). Identifying which components of physical fitness are associated with HRQoL in people with fibromyalgia may contribute to the development of more specific therapeutic strategies.
Objective: The 2 aims of this study were to examine the association of different components of physical fitness (ie, flexibility, muscle strength, speed and agility, and cardiorespiratory fitness) with HRQoL and to determine the extent to which any association between the components of physical fitness and HRQoL were of clinical relevance to women with fibromyalgia.
Design: A cross-sectional design was used.
Methods: This study included 466 women with fibromyalgia from southern Spain (Andalusia). The Senior Fitness Test battery and the handgrip test were used to assess physical fitness, and the 36-Item Short-Form Health Survey (SF-36) was used to assess HRQoL. Tender points, cognitive impairment, anthropometric measurements, and medication usage were also measured. First, multivariate linear regression was used to assess the individual relationship of each physical fitness test with the 8 dimensions of the SF-36. Second, a standardized composite score was computed for each component of physical fitness (flexibility, muscle strength, speed and agility, and cardiorespiratory fitness). A 1-way analysis of covariance to assess the differences in each of the 8 dimensions of the SF-36 across each physical fitness composite score was conducted. Forward stepwise regression was performed to analyze which components of physical fitness were independently associated with the SF-36 physical and mental component scales.
Results: Overall, higher levels of physical fitness were associated with higher levels of HRQoL (regardless of the SF-36 subscale evaluated). The effect sizes for HRQoL between participants with the lowest and the highest physical fitness levels ranged from moderate to large (Cohen d = 0.53–0.90). The muscle strength composite score was independently associated with the SF-36 physical component scale, whereas the flexibility composite score and cardiorespiratory fitness were independently associated with the SF-36 mental component scale.
Limitations: A limitation was that the cross-sectional design precluded the establishment of causality. Additionally, only women were included in the study, because fibromyalgia predominantly affects women.
Conclusions: High levels of physical fitness were consistently associated with better HRQoL in women with fibromyalgia; clinically relevant differences were demonstrated between those at extreme physical fitness levels. Muscle strength, flexibility, and cardiorespiratory fitness were independent indicators of HRQoL.
