Activité physiqueRésumé : 1. Il existe des effets positifs de la réadaptation à la marche chez les patients atteints d’artériopathie oblitérante des membres inférieurs (AOMI) qu’ils soient symptomatiques ou non.
2. L’ischémie des membres inférieurs induite lors de l’exercice de marche peut être nécessaire pour améliorer les performances de marche chez des personnes atteintes d’AOMI.
3. Activité physiqueTags : Paralysie cérébrale Marche Activité physique Renforcement musculaire Résumé : Objectif : La paralysie cérébrale est l’atteinte motrice la plus fréquente chez l’enfant, avec une prévalence allant de 1,5 à 3,8 pour 1000 naissances selon les pays. De nombreuses études mettent en avant l’avantage de la pratique d’activité physique dans la paralysie cérébrale. Nous étudions ici son impact sur les paramètres spatio-temporels de la marche.
Matériel et méthode : La recherche a été effectuée sur PubMed avec les mots-clés « Cerebral Palsy », « Exercises » et « Walking ». Finalement, 49 articles ont été retenus et cinq ont été sélectionnés pour être étudiés en détail. La population étudiée est composée d’enfants et adolescents (six à seize ans) atteints de paralysie cérébrale spastique. L’intervention ciblée est le renforcement musculaire du bas du corps par divers exercices avec ou sans matériel spécifique.
Résultats : Les échantillons vont de 15 à 30 enfants, âgés de six à seize ans. Les scores GMFCS sont de I à IV. Les exercices sont proposés trois à cinq fois par semaine avec une durée d’exercice de 25 à 70 min par session. Les études ont été menées pendant six à douze semaines. Différentes échelles de mesures ont été utilisées, avec une mesure avant et après l’intervention. Deux études ont utilisé une troisième mesure à un mois après la fin de l’intervention.
Conclusion : Une amélioration des paramètres spatio-temporels de la marche a été observée dans toutes les études, plus spécifiquement pour la vitesse de marche, la longueur de pas et la cadence de marche. Les limites de ces études sont que les échantillons sont petits et très hétérogènes. Activité physiqueRésumé : Cardiac surgery via median sternotomy is performed in over 1 million patients per year worldwide. Despite evidence, sternal precautions in the form of restricted arm and trunk activity are routinely prescribed to patients following surgery to prevent sternal complications. Sternal precautions may exacerbate loss of independence and prevent patients from returning home directly after hospital discharge. In addition, immobility and deconditioning associated with restricting physical activity potentially contribute to the negative sequelae of median sternotomy on patient symptoms, physical and psychosocial function, and quality of life. Interpreting the clinical impact of sternal precautions is challenging due to inconsistent definitions and applications globally. Following median sternotomy, typical guidelines involve limiting arm movement during loaded lifting, pushing, and pulling for 6 to 8 weeks. This perspective paper proposes that there is robust evidence to support early implementation of upper body activity and exercise in patients recovering from median sternotomy while minimizing risk of complications. A clinical paradigm shift is encouraged, one that encourages a greater amount of controlled upper body activity, albeit modified in some situations, and less restrictive sternal precautions. Early screening for sternal complication risk factors and instability followed by individualized progressive functional activity and upper body therapeutic exercise is likely to promote optimal and timely patient recovery. Substantial research documenting current clinical practice of sternal precautions, early physical therapy, and cardiac rehabilitation provides support and the context for understanding why a less restrictive and more active plan of care is warranted and recommended for patients following a median sternotomy. There is accumulating evidence to suggest that OA is an inflammatory disease of the entire synovial joint and has multiple phenotypes. This presents the OA research community with new challenges and opportunities. The main challenge is to understand the root cause of the disease and identify differences and similarities between OA phenotypes. The key opportunity is the possibility of developing personalized and individualized prevention and treatment strategies for OA patients with different phenotypes of the disease. Indeed, it has been suggested that this is the era of ‘personalized prevention’ for OA. The aim of this mini-review paper is to focus on the pathophysiological aspects of OA development and progression, review the current concepts and discuss the future of personalized medicine for OA.
Method: The PubMed/MEDLINE bibliographic database was searched using the keywords ‘pathophysiology’ and ‘osteoarthritis’.
Results: The PubMed/MEDLINE search yielded more than 12,000 relevant papers. A selection of these papers is reviewed here.
Conclusion: There has been slow but steady progress in our understanding of the pathophysiology of OA over the last two decades. However, large gaps remain in our knowledge of OA pathogenesis and this impacts negatively on patients and drug development pipeline.
