EnfantTags : Rhythm Music Syntax Cochlear implant Temporal attention Rehabilitation Résumé : Objective: Previous research has suggested the use of rhythmic structures (implemented in musical material) to improve linguistic structure processing (i.e., syntax processing), in particular for populations showing deficits in syntax and temporal processing (e.g., children with developmental language disorders). The present study proposes a long-term training program to improve syntax processing in children with cochlear implants, a population showing syntax processing deficits in perception and production.
Method: The training program consisted of morphosyntactic training exercises (based on speech processing) that were primed by musical regular primes (8 sessions) or neutral baseline primes (environmental sounds) (8 sessions). A crossover design was used to train 10 deaf children with cochlear implants. Performance in grammatical processing, non-word repetition, attention and memory was assessed before and after training.
Results: Training increased performance for syntax comprehension after both prime types but for grammaticality judgements and non-word repetition only when musical primes were used during training. For the far-transfer tests, some effects were also observed for attention tasks, especially if fast and precise sequential analysis (sequencing) was required, but not for memory tasks.
Conclusion: The findings extend the previously observed beneficial short-term effects of regular musical primes in the laboratory to long-term training effects. Results suggest that the musical primes improved the processing of the syntactic training material, thus enhancing the training effects on grammatical processing as well as phonological processing and sequencing of speech signals. The findings can be interpreted within the dynamic attending theory (postulating the modulation of attention over time) and associated oscillatory brain activity. EnfantTags : Induced-pain Motor disability Rehabilitation Children Résumé : Background: Pain is one of the symptoms reported most by children with motor disabilities particularly during daily living activities in institutions and during rehabilitation. Despite the care and consideration of professionals, a wide range of motor and cognitive disabilities, limited communication skills, the presence of chronic pain and frequent care interventions place such children at high risk of experiencing induced pain.
Objectives: We aimed to identify care-related pain and discomfort in children with motor disabilities in rehabilitation centres and the characteristics of children at risk of induced pain. A further aim was to evaluate the validity of a method for the continuous assessment of care-related pain.
Methods: Patients were recruited from 2 paediatric rehabilitation centres. The level of pain or discomfort experienced during each daily care activity was evaluated for 5 days and 1 night by using the FLACC-r scale and a visual analog scale (VAS) rated by the caregiver (VAS caregiver) and the patient (VAS patient).
Results: We included 32 children (mean age: 8.5±5 years, range: 1–15 years) with 1302 care activities evaluated. Overall, 3.6% of the activities were rated as painful and 11% uncomfortable. The most frequent painful activities were mouth care, transfers standing and dressing. The most frequent uncomfortable activities were passive limb mobilisation, dressing and transfers. Children with neurological disorders were at increased risk of induced pain.
Conclusions: Children with motor disabilities experienced pain during daily care activities. EnfantRésumé : Background: Caregiver-provided physical therapy home programs (PTHP) play an important role in enhancing motor outcomes in pediatric patient populations.
Purpose: This scoping review systematically mapped clinical trials of caregiver–provided PTHP that were aimed at enhancing motor outcomes in children who have or who are at risk for motor delay, with the purpose of (1) describing trial characteristics; (2) assessing methodologic quality; and (3) examining the reporting of caregiver-related components.
Data Sources: Physiotherapy Evidence Database (PEDro), Cochrane CENTRAL, PubMed, Scopus, ScienceDirect, ProQuest Central, CINAHL, LILACS, and OTseeker were searched up to July 31, 2017.
Study Selection: Two reviewers independently assessed study eligibility. Randomized or quasi-randomized controlled trials on PTHP administered by parents, other family members, friends, or informal caregivers to children who had or who were at risk for motor delay were included.
Data Extraction: Two reviewers independently appraised trial quality on the PEDro scale and extracted data.
Data Synthesis: Twenty-four articles representing 17 individual trials were identified. Populations and interventions investigated were heterogeneous. Most of the trials had important research design limitations and methodological issues that could limit usefulness in ascertaining the effectiveness of caregiver-provided PTHP. Few (4 of 17) trials indicated involvement of caregivers in the PTHP planning, assessed how the caregivers learned from the training or instructions provided, or carried out both.
Limitations: Included studies were heterogeneous, and unpublished data were excluded.
Conclusions: Although caregiver-provided PTHP are important in addressing motor outcomes in this population, there is a lack of evidence at the level of clinical trials to guide practice. More research is urgently needed to determine the effectiveness of care-giver-provided PTHP.
