Masseur kinésithérapeuteRésumé : Background: In the management of chronic low back pain (LBP), identifying and managing more patients who are at high risk and who have psychological barriers to recovery is important yet difficult.
Objective: The objective of this study was to test physical therapists’ ability to allocate patients into risk stratification groups, test correlations between therapists’ assessments of psychological factors and patient questionnaires, and explore relationships between psychological factors and therapists’ self-reported competence to manage patients with chronic LBP.
Design: This was a pragmatic, observational study.
Methods: Patients completed the STarT Back Tool (SBT, for risk stratification), the Four-Dimensional Symptom Questionnaire (distress, depression, anxiety), and the Tampa Scale of Kinesiophobia (kinesiophobia) prior to the intake session. After this session, physical therapists estimated patient prognostic risk using the 3 SBT categories and rated patient psychological factors using a 0-to-10 scale. Finally, therapists reported their self-reported competence to manage the patient. Intraclass and Spearman rank correlations tested correlations between therapists’ intuitive assessments and patient questionnaires. A linear-mixed model explored relationships between psychological factors and therapists’ self-reported competence.
Results: Forty-nine patients were managed by 20 therapists. Therapists accurately estimated SBT risk allocation in only 41% of patients. Correlations between therapist perceptions and patient questionnaires were moderate for distress (r = 0.602) and fair for depression (r = 0.304) and anxiety (r = 0.327). There was no correlation for kinesiophobia (r = −0.007). Patient distress was identified as a negative predictor of therapists’ self–reported competence.
Limitations: This was a cross-sectional study, conducted in only 1 center.
Conclusions: Physical therapists were not very accurate at allocating patients into risk stratification groups or identifying psychological factors. LectureRésumé : Devenir lecteur, c’est toujours faire l’expérience du changement : franchir le seuil entre la petite enfance et l’âge de raison, explorer d’autres mondes, changer d’époque, investir d’autres vies que la sienne, incorporer d’autres visions du monde. Cette « occupation » (j’occupe le territoire du texte autant qu’il occupe mon esprit) peut avoir des répercussions durables. C’est pourquoi les livres furent parfois considérés avec méfiance. On les soupçonnait de rendre soit mous, soit fous. Les temps ont changé. La littérature apparaît de plus en plus comme un outil de connaissance, de développement personnel et d’émancipation. En nous plongeant dans des univers sociaux et mentaux, elle élargit notre compréhension de l’histoire, de la sociologie, de la psychologie. Les philosophes la parent de vertus morales : elle développerait notre empathie et notre tolérance. Elle aurait même le pouvoir de réparer les âmes blessées et les sociétés morcelées par les crises, en ressoudant par le verbe la communauté des vivants. Comment et jusqu’où la littérature agit-elle ? PhilosophieRésumé : L’humain est un animal songeur. De ses premiers balbutiements à son dernier souffle, il possède une prodigieuse capacité à rêver le monde. Tout, chez lui, en lui, se transforme en images mentales : les idées, les émotions, les lieux, le désir, la morale, les souvenirs, les projets. « Le virtuel est la vertu de l’homme », écrit joliment Michel Serres dans son dernier livre, Le Gaucher boiteux (Le Pommier, 2015).
L’imaginaire n’est donc pas le refuge de chimères que l’on a si longtemps cru, et qui tiendrait les rêveurs à l’écart des réalités. Une révolution conceptuelle est en train de balayer les anciennes dichotomies philosophiques : imagination contre raison, imaginaire contre réel, virtuel contre actuel. Avec elle émerge une nouvelle conception de l’imaginaire, désormais perçu comme une dynamique par laquelle un sujet entre en contact avec le monde, y œuvre et s’y construit.
Ce dossier tire les fils de cette redéfinition, qui est en même temps une revalorisation. Des amis imaginaires de la petite enfance jusqu’aux grands mythes qui racontent l’identité rêvée des peuples, l’imaginaire irrigue tous les champs de l’action et de la pensée. MalnutritionTags : TUG Dual task TUG Falls Older age Malnutrition Handgrip strength Frailty Résumé : Background: The cognitive timed up-and-go dual task (CogTUG) has been proposed to improve the performance of the timed up-and-go (TUG) test for predicting falls in older patients and as a screening tool for early detection of frailty. We aimed to determine whether the CogTUG score is associated with a history of falls in frail older outpatients with gait disorders.
Methods: This retrospective study involved outpatients >75 years old with or without previous falls who were admitted from 2012 to 2014 to a geriatric day hospital for gait disorders. Patients took the TUG and CogTUG tests on the day of comprehensive geriatric assessment.
Results: Among the 161 patients included (157 analyzed; mean age 84.4±6.2 years; 72% women), 84 (53.5%) had fallen in the previous year: 105 (66.9%) were considered pre-frail and 52 (33.1%) frail. As compared with non-fallers, fallers had lower Tinetti balance scores (P= 0.0004) and handgrip strength (P= 0.03), more lost weight (P= 0.04), and they took longer to perform the TUG test (P= 0.04). Fallers and non-fallers did not differ in time taken to perform the CogTUG test (30.7±11.2 vs. 28.5±10.2s, P= 0.20). History of falls was associated with only weight loss (odds ratio 3.43; 95% CI 1.13–11.30, P= 0.03) and handgrip strength (0.88; 0.78–0.97, P= 0.02) on multivariate analysis.
Conclusion: Unlike TUG scores, the CogTUG score was not associated a history of falls in frail older outpatients with gait disorders.
