FamilleTags : Activité physique Famille Paralysie cérébrale Pédiatrie Réhabilitation Résumé : La paralysie cérébrale est la principale cause du handicap moteur de l’enfant. Les lésions cérébrales surviennent sur un cerveau en développement, elles ne sont pas évolutives mais définitives. Les déficiences liées à la posture et au mouvement étant au premier plan, les enfants atteints de paralysie cérébrale ont alors, le plus souvent, besoin de kinésithérapie tout au long de leur vie. Or, au cours de leur parcours de soin jalonné par de nombreuses interventions multidisciplinaires, ces jeunes perdent parfois leur motivation et le sens de leurs séances de kinésithérapie. Proposer une rééducation autrement, sous forme d’activités physiques attractives, ludiques et valorisantes nous permettrait d’être plus pertinents dans nos pratiques et de nous inscrire dans les recommandations de promotion de l’activité physique chez ces patients. D’autant plus que la plupart de ces jeunes peinent, de par leurs déficiences neuromotrices, à suivre ces indications.

Norme soinsTags : Co-détermination d’objectifs Entraînement dirigé vers l’objectif Kinésithérapie Paralysie Cérébrale Recommandations de Bonnes Pratiques Réhabilitation Résumé : L’édition des Recommandations de Bonnes Pratiques (RBP) fait suite à une demande réalisée auprès de la HAS par les personnes concernées par la paralysie cérébrale (PC). Les RBP soulignent la nécessité de mettre en œuvre un partenariat, mais en pratique il n’est pas toujours simple d’aboutir à une relation équilibrée entre personnes avec PC, leurs proches/aidants et les professionnels de rééducation. La co-détermination récurrente d’objectifs de qualité est cruciale afin de faciliter la mise en place des thérapies et d’assurer le maintien de la motivation et de l’engagement tout au long de la prise en charge. Dans cette perspective, les RBP apportent un éclairage pour sélectionner les moyens réadaptatifs constituant le plan d’action individualisé co-déterminé. Ainsi, en cas d’identification de facteurs de risque de troubles musculo-squelettiques secondaires, il est recommandé de proposer des postures passives nocturnes dans le but de prévenir et/ou retarder l’apparition de rétractions musculaires et de ne pas proposer d’étirements passifs chez les enfants et adolescents avec PC. Afin d’améliorer l’activité des membres supérieurs des personnes présentant une PC unilatérale ou bilatérale, les programmes de rééducation intensive, incluant les principes de l’apprentissage moteur et intégrant des interventions ciblant spécifiquement des objectifs fonctionnels de la vie quotidienne sont priorisés. Afin d’améliorer la fonction motrice globale des enfants avec PC, l’entraînement à la marche est préconisé. Enfin, il est recommandé de proposer des activités physiques et des activités sportives afin de prévenir la désadaptation à l’effort des enfants, adolescents et adultes avec PC. Les RBP de la HAS sont construites à partir des études issues de la littérature scientifique internationale. Ces études s’intéressent particulièrement aux enfants et adolescents ayant des atteintes motrices légères à modérées. La proposition de recommandations sur une population plus large n’est pour l’instant pas possible. Activité physiqueTags : Osteoarthritis Rehabilitation Exercise Strength training Evidence-based medicine Résumé : Rehabilitation is widely recommended in national and international guidelines for managing osteoarthritis (OA) in primary care settings. According to the 2014 OA Research Society International (OARSI) recommendations, rehabilitation is even considered the core treatment of OA and is recommended for all patients. Rehabilitation for OA widely includes land- and water-based exercise, strength training, weight management, self-management and education, biomechanical interventions, and physically active lifestyle. We performed a critical narrative review of the efficacy and safety of rehabilitation for managing OA and discuss evidence-based international recommendations. The process of article selection was unsystematic. Articles were selected based on authors’ expertise, self-knowledge, and reflective practice. For the purpose of the review, we focused on land- and water-based exercise and strength training for knee, hip and hand OA. Other aspects of rehabilitation in OA are treated elsewhere in this special issue. Exercise therapy is widely recommended for managing knee, hip and hand OA. However, the level of evidence varies according to OA location. Overall, consistent evidence suggests that exercise therapy and specific strengthening exercise or strength training for the lower limb reduce pain and improve physical function in knee OA. Evidence for other OA sites are less consistent. Therefore, because of the lack of specific studies, recommendations for hip and hand OA are mainly derived from studies of knee OA. In addition, no recommendations have been established regarding the exercise regimen. The efficacy and safety of exercise therapy and strength training need to be further evaluated in randomized controlled trials of patients with hip and hand OA. The optimal delivery of exercise programs also has to be more clearly defined. Membre supérieurTags : Stroke Upper limb Video games Rehabilitation Résumé : Background: Few rehabilitation methods have proven their efficacy in increasing sensori-motor recovery and/or function of the upper limb (UL) after stroke. Video games (VGs) are promising tools in this indication.
Objective: To compare UL rehabilitation by using VGs and conventional rehabilitation (CR) in patients with sub-acute stroke.
Design: Single-blind, multicentric trial, with central randomization and stratification by center.
Setting: Physical and rehabilitation medicine departments of 2 university hospitals.
Participants: Adults within 3 months after a first vascular cerebral accident, with UL Fugl Meyer Score (UL-FMS)<30/66 and without major cognitive impairment.
Intervention: A 45-min additional session of conventional occupational therapy (OT) or a VG-based OT session as add-on therapy to usual rehabilitation programs, 5 days/week for 6 weeks.
Main Outcome Measures: Primary outcome: UL-FMS. Secondary outcome: Box and Block Test (BBT), Wolf Motor Function test (WMFT), Motor Activity Log (MAL), Barthel Index and quality of life (SF-36).
Results: We included 51 patients (20 women) at a mean (SD) of 27.2 (19.4) days post-stroke (mean age 58 years [range 24–83]), 26 in the CR group and 25 in the VG group (23 in each group at 6-month follow-up). The mean duration of the additional rehabilitation session was similar in both groups: 29.3 (4.3) vs 28.0 (4.4) min in CR and VG groups. Shoulder pain occurred in 4 patients in the VG group versus 7 in the CR group. At day 45, gain in UL-FMS did not significantly differ between the groups (CR mean 17.8 [14.6] vs VG 24.1 [14.8]; P =0.10), whereas gain in BBT was doubled in the VG group (CR 7.4 [12.2] vs VG 15.7 [16.3]; P =0.02). At 6-month follow-up, the study was inconclusive about between-group differences in UL-FMS, BBT and other criteria. Post-hoc analysis showed that gains in UL-FMS or BBT were significantly higher in the VG than CR group for patients included within 30 days post-stroke.
Conclusion: In general, we cannot conclude that video gaming and conventional OT led to different long-term sensorimotor recovery of the UL after sub-acute stroke.
