GenouTags : Knee osteoarthritis Orthosis Braces Decision-making tool Guidelines Résumé : Objective: To develop a decision-making tool (DMT) to facilitate the prescription of knee orthoses for patients with osteoarthritis (OA) in daily practice.
Methods: A steering committee gathered a multidisciplinary task force experienced in OA management/clinical research. Two members performed a literature review with qualitative analysis of the highest-quality randomized controlled trials and practice guidelines to confirm evidence concerning knee orthosis for OA. A first DMT draft was presented to the task force in a 1-day meeting in January 2016. The first version of the DMT was criticized and discussed regarding everyday practice issues. Every step was discussed and amended until consensus agreement was achieved within the task force. Then 4 successive consultation rounds occurred by electronic communication, first with primary- and secondary-care physicians, then with international experts. All corrections and suggestions by each member were shared with the rest of the task force and included to reach final consensus. The final version was validated by the steering committee.
Results: The definition and indication of several types of knee orthoses (sleeve, patello-femoral, hinged or unicompartmental offloading braces) were detailed. Orthoses may be proposed in addition to first-line non-pharmacological treatment if patient acceptance is considered good. At every step, a specific clinical assessment is needed.
Discussion/conclusion: Based on the latest high-level evidence, practice guidelines, and an expert panel, a DMT to facilitate daily practice prescription of knee orthoses for OA patients was designed. It remains nevertheless insufficiently known and defined. The objective of this work was to develop precise recommendations for caregivers and relatives.
Method: The elaboration of these guidelines followed the procedure validated by the French health authority for good practice recommendations, close to the Prisma statement, involving a systematic, critical review of the literature and the expert opinions of the French Society of Physical Medicine and Rehabilitation (SOFMER) group.
Results: 458 articles were identified, among which 98 were selected for their relevance to the theme of the research. None of the studies reached the highest level of evidence. Fifteen controlled studies reached a relatively high level of evidence (level 2); other studies were case series or expert opinions, and other articles again were reviews of the literature and theoretical points of view. The holistic approach structured into programmes, cognitive-behavioural therapy, and family and systemic therapy, despite the low levels of proof, are recommended in first intention at all stages in the evolution of TBI. Relational and adaptive approaches, rehabilitation and vocational approaches, and psychoanalytical therapies may be useful, provided that therapists are familiar with and trained in traumatic brain injury.
Conclusion: Despite the small number of publications and a low level of proof, a number of recommendations for the non-pharmacological approach to psychological and behavioural disorders in TBI is proposed by the consensus conference of experts. LombalgieTags : Guidelines Pre-return-to-work medical consultation Chronic low back pain Job retention Résumé : Objectives: The pre-return-to-work medical consultation during sick leave for low back pain (LBP) aims at assessing the worker's ability to resume working without risk for his/her health, and anticipating any difficulties inherent to returning to work and job retention. This article summarizes the good practices guidelines proposed by the French Society of Occupational Medicine (SFMT) and the French National Health Authority (HAS), and published in October 2013.
Methods: Good practices guidelines developed by a multidisciplinary and independent task force (24 experts) and peer review committee (50 experts) based on a literature review from 1990 to 2012, according to the HAS methodology.
Results: According to the labour regulations, workers can request a medical consultation with their occupational physician at any time. The pre-return-to-work consultation precedes the effective return-to-work and can be requested by the employee regardless of their sick leave duration. It must be scheduled early enough to: (i) deliver reassuring information regarding risks to the lower back and managing LBP; (ii) evaluate prognostic factors of chronicity and prolonged disability in relations to LBP and its physical, social and occupational consequences in order to implement the necessary conditions for returning to work; (iii) support and promote staying at work by taking into account all medical, social and occupational aspects of the situation and ensure proper coordination between the different actors.

Objectifs : Déterminer quels dispositifs locaux en dehors des supports et quels médicaments utiliser pour assurer le bourgeonnement et la réépidermisation d’une escarre en 2012.
Méthode : Revue systématique de la littérature avec interrogation des bases de données PASCAL Biomed, PubMed et Cochrane Library entre 2000 et 2010.
Résultats : Les articles analysés traitent peu spécifiquement du bourgeonnement et de la réépidermisation des escarres. Les données qui permettent de préférer certains types de pansements modernes à d’autres restent d’un faible niveau de preuve. Les études sont en revanche plus significatives sur la comparaison de l’intérêt des hydrocolloïdes par rapport aux gazes imbibées.
Discussion : Les études sont de faible puissance avec des résultats et des populations hétérogènes et des objectifs différents rendant difficile une méta-analyse. Des travaux cliniques supplémentaires, de qualité méthodologique élevée, sont nécessaires pour élaborer des recommandations plus précises.

Objectifs : Déterminer quels dispositifs locaux en dehors des supports et quels médicaments utiliser en matière de prévention d’escarre(s) en 2012.
Méthode : Revue systématique de la littérature avec interrogation des bases de données PASCAL Biomed, PubMed et Cochrane Library entre 2000 et 2010.
Résultats : Les soins de nursing incluant l’utilisation de produits doux, non irritants pour le lavage, l’hydratation de la peau avec des émollients, la protection des peaux fragilisées par l’incontinence par application d’un protecteur cutané et l’application de pansements en regard des proéminences osseuses pour réduire les forces de cisaillement restent d’actualité (Grade C).
Discussion : Les soins de nursing et l’utilisation de pansements chez les patients à risque d’escarre sont du ressort du rôle propre infirmier. La mobilisation des équipes soignantes passe par le dépistage des facteurs de risque et par la connaissance des médicaments et dispositifs locaux.
